Angel Intuitive Massage

Please print and submit a signed copy of the Consent and Policies with Client Intake Forms, at our request, prior to an Angel Intuitive Massage session. A completed Client Intake Form is required for each massage therapy session. Adult supervision combined with a signed Minor Consent Forms will be required for all individuals under the age of 18. Ka Malama `Anela reserves the right to revoke the privilege of massage therapy provided to minors at any time.
